Erysipelas is a cellulitis caused by a toxin of Strep. pyogenes and occasionally by streptococci of groups B, C and D.11,12 It is characterized by an abrupt onset of fiery red swelling of the face or extremities. Distinctive features are well-defined margins, particularly along the nasolabial fold, rapid progression and intense pain (Figure 10-9). Flaccid superficial bullae may develop during the second to third day of the illness, but extension to deeper soft tissues is rare. Since it is a toxic reaction, cultures of skin biopsies are generally negative, though throat cultures may be positive.
Belladonna is often the first remedy considered for acute erysipelas with sudden onset, severe redness, and swelling. It is particularly useful when the affected area is hot, swollen, and shiny, with a raised, bright red border.
Key symptoms: Red, inflamed skin with a burning sensation and fever. The individual may feel delirious, thirsty, or agitated. The affected area is usually hot to touch, and the person may experience throbbing pain in the inflamed region.
Rhus Tox is indicated for erysipelas with itching, burning, and stinging pain, especially when the eruption is accompanied by blisters or a shiny, red appearance. It is particularly useful when the condition arises from exposure to cold, damp weather or after overexertion.
Key symptoms: Intense itching and burning in the affected skin. The rash may feel tight and the person may experience a sense of restlessness or agitation. There may also be joint pain or muscle stiffness, especially after initial movement or rising from rest.
Hepar Sulph is helpful for erysipelas with pus formation, sensitive skin, and suppurating lesions. It is especially useful when the condition is painful and inflamed, and the person has a tendency to develop abscesses.
Key symptoms: Intense sensitivity to touch and pain, with the skin appearing red, hot, and inflamed. There may be suppuration or pus in the affected area, and the individual may feel irritable, sensitive, or cold despite the fever.
Apis is indicated for erysipelas with swelling, redness, and a stinging pain that is worse from heat and better from cold. It is particularly helpful when the skin is swollen, edematous, and there is a sensation of burning or stinging.
Key symptoms: Swelling with a pale, shiny, or bluish-red appearance of the skin, especially in the face or limbs. The skin may feel hot, but there is a preference for cold compresses or cool air. The pain may be stinging or burning, and there may be marked tenderness.
Mercurius is useful for erysipelas with swelling, heat, and persistence of the infection. It is particularly helpful when there is a sharp, burning pain and the skin looks dark red, with a tendency to break out in pustules or ulcers.
Key symptoms: Fever, sweating, and intense inflammation. The skin is swollen, with a dark red color and pustular eruptions. There may be a feeling of exhaustion, nausea, and a foul-smelling discharge from the affected area.
Arsenicum is often indicated when erysipelas is associated with weakness, anxiety, and restlessness. It is particularly useful for individuals who are exhausted and feel better from warmth but have a worsening condition at night.
Key symptoms: Burning pain with great restlessness and extreme exhaustion. The individual may feel worse after midnight, and the skin may be hot and red with extreme sensitivity. There is a strong desire for warmth and frequent thirst, often with vomiting or diarrhea.
Sulphur is useful when erysipelas is accompanied by itching, burning, and dryness. It is especially helpful when the rash is chronic or recurring and the individual has a history of skin issues such as eczema or psoriasis.
Key symptoms: Intense itching and burning in the skin, particularly with a dry or scaly appearance. The skin is often hot, inflamed, and irritated. The person may have a general feeling of heat, with a desire to scratch the inflamed area.
Ledum is indicated for erysipelas that is accompanied by swelling, dark red patches, and discoloration, often with pitting edema. The condition may improve with cold applications and worsen with heat.
Key symptoms: Red, swollen, and discolored skin, especially in the legs or feet, which may appear dark red or purplish. The swelling may feel cold and there is improvement from cold compresses or cooler temperatures.
Natrum Muriaticum is often used when erysipelas is accompanied by emotional causes, such as grief, stress, or sensitivity. It is particularly helpful when the rash is marked by swelling, dark redness, and blister formation.
Key symptoms: Inflamed, swollen skin with dark red or bluish areas. The person may also experience headache, nausea, and grief. There may be blisters or vesicles on the skin, and the person may feel better in open air.
Kali bichromicum is indicated for erysipelas that involves the nasal region, often causing ulcerations or scabbing. It is useful when there is thick, yellowish discharge or a sluggish, slow recovery from the infection.
Key symptoms: Swelling and pain in the skin, particularly on the face or nose, with a tendency to form thick, yellowish discharge or crusts. The skin may be deep red, and the person may have a feeling of general weakness or exhaustion.
