Benign Paroxysmal Positional Vertigo (BPPV) is one of the most common causes of vertigo, characterized by brief episodes of dizziness or spinning sensation that occur with changes in head position — such as turning over in bed, looking up, or bending down.
It results from a mechanical problem in the inner ear (vestibular system). Tiny calcium crystals known as otoliths (canaliths) become dislodged from their normal location (utricle) and move into one of the semicircular canals. When the head changes position, these crystals stimulate hair cells abnormally, sending false signals of movement to the brain — causing vertigo.
Although BPPV is not life-threatening, it can be distressing and disabling, especially in older adults.
Common causes and risk factors include:

Remedies are chosen according to modalities (triggers, aggravations, accompanying symptoms) and personal emotional-physical traits.

Remedies such as Conium maculatum, Cocculus indicus, Bryonia alba, Gelsemium, and Belladonna are often indicated, depending on symptom characteristics.
Conium maculatum is the leading remedy for BPPV. Vertigo is worse on turning the head, lying down, or rising from bed, and may be accompanied by nausea and faintness. The patient feels as if the bed is moving or the room is spinning. Motion aggravates, rest relieves.
👉 Best indicated in age-related BPPV or vertigo from inner ear degeneration.
Cocculus is indicated when vertigo is associated with nausea, weakness, or vomiting, especially on motion, traveling, or lack of sleep. The patient feels giddy, heavy-headed, and exhausted, with a sensation of floating or swaying.
👉 Useful for motion-induced vertigo or vestibular imbalance aggravated by fatigue or travel.
Bryonia suits cases where vertigo worsens on the least movement and improves when the head is kept perfectly still. The dizziness is accompanied by nausea, headache, and dryness of mouth. Even turning the eyes or changing position increases the spinning.
👉 Indicated when vertigo arises from dehydration, inflammation, or heat exposure.
Gelsemium is indicated for vertigo with general weakness, heaviness of eyelids, and trembling. The patient feels light-headed, dull, and unsteady, as if intoxicated. It often occurs after emotional stress, anticipation, or nervous exhaustion.
👉 Excellent for nervous or anxiety-related vertigo and imbalance.
Belladonna is useful when vertigo comes suddenly, with throbbing pain, redness of face, and heat in the head. The person may feel as if falling backward or to one side. Sudden movements of the head bring on violent spinning and nausea.
👉 Indicated for acute congestive vertigo with strong circulatory disturbance.
Silicea is indicated when vertigo occurs on rising from sitting, looking upward, or bending backward, often with trembling and weakness. The patient feels cold, chilly, and easily fatigued.
👉 Suited to delicate, nervous individuals with chronic or recurrent BPPV.
Tabacum is indicated for vertigo accompanied by nausea, vomiting, cold sweat, and death-like pallor. The dizziness is worse from motion and warmth, and better from fresh air or closing eyes.
👉 For severe vestibular vertigo with nausea resembling motion sickness.
Phosphorus is indicated when vertigo is associated with visual disturbances, especially blurred or double vision. The patient may feel as if everything were moving in circles, with weakness and anxiety.
👉 Useful in chronic vestibular disorders or visual-vestibular imbalance.
Nux vomica is suited for vertigo from overwork, stress, or stimulants (coffee, alcohol). The dizziness occurs on bending backward, after meals, or on waking, and is accompanied by irritability and nausea.
👉 For BPPV triggered or aggravated by mental strain and lifestyle imbalance.
