Hydrocele is an abnormal accumulation of serous fluid within the tunica vaginalis surrounding the testis or along the spermatic cord, leading to painless scrotal swelling. It commonly affects infants (congenital type) due to a patent processus vaginalis, or adults (acquired type) secondary to trauma, infection, inflammation, or obstruction of lymphatic drainage. The swelling is usually soft, non-tender, and translucent on transillumination. Large hydroceles can cause discomfort, heaviness, and difficulty in walking or sitting. Diagnosis is clinical, supported by ultrasound to exclude hernia or testicular pathology.
