An anal fistula is an abnormal passageway that develops from inside your anus to the skin outside. It usually develops in the upper part of your anus (butthole), where your anal glands are. When these glands become infected, drainage from the infection can create a fistula. This infection is called a perianal abscess. (Sometimes an anal fistula is also called a perianal fistula. “Perianal” means in the region of your anus.)
A fistula is a relatively common anorectal condition. It’s twice as common in men and people assigned male at birth (AMAB). About half of people who get an infected anal gland will develop a fistula. An infected gland that forms an abscess, a pocket of pus that needs to drain, causes 75% of anal fistulas.
Graphites is often recommended for chronic, slow-healing conditions like anal fistula. It is particularly useful when there is a thick, sticky discharge and a tendency for the condition to worsen in cold weather.
Key symptoms: Thick, sticky discharge from the fistula; painful, hard, and deep cracks or ulcers; tendency to develop slow-healing wounds.
Hepar Sulph is often used when there is a tendency for infection to occur, and the fistula is painful, with a feeling of extreme sensitivity to touch.

Silicea is useful for individuals who have a slow recovery from infections or wounds. It helps to drain the infection and stimulate healing in cases of chronic fistulas.

Arsenicum is often used when the fistula is accompanied by restlessness, anxiety, and burning pain. The discharge may be foul-smelling, and the person may feel generally weak and unwell.

Mercurius is helpful for fistulas that are inflamed and painful, often with a purulent discharge. This remedy is particularly useful when there is a tendency for the fistula to be infected and prone to recurrence.
Key symptoms: Painful, swollen, inflamed fistula with greenish or yellowish pus; excessive salivation; offensive odor; sensitivity to warmth and cold.
Kali Bichromicum is used when there is a thick, stringy discharge from the fistula that is difficult to remove. The condition is often worse in the morning.
Key symptoms: Thick, stringy discharge from the fistula; pain or burning sensation in the area; aggravation of symptoms in the morning.
Lachesis can be helpful for fistulas that are associated with a feeling of congestion and inflammation. This remedy is suitable for individuals who have hot, red, and swollen areas around the anus.
Key symptoms: Hot, swollen, and congested areas around the anus; a tendency for the fistula to discharge excessively, often with a dark or foul-smelling fluid; sensitivity to touch.
This remedy is particularly useful if the fistula is associated with emotional stress or suppressed grief. It helps to alleviate the physical symptoms while addressing the emotional component.
Key symptoms: Fistula with watery or foul-smelling discharge; tendency to hide feelings or emotions; irritation or pain aggravated by stress.
Nux Vomica is useful for fistulas associated with digestive issues, especially when there is constipation or difficulty with bowel movements. It is also helpful for individuals who are irritable or overworked.

Sulphur is a common remedy for chronic conditions, including fistulas, where there is itching, burning, and a tendency for the skin to be rough and unhealthy.

Thuja is helpful when the fistula is associated with growths or warts around the anal area. It can also be beneficial when there is a history of suppressed infections.
Key symptoms: Growths or warts near the anus; thick, purulent discharge; the fistula may worsen after taking antibiotics or other suppressive treatments.
Baryta Carbonica is often indicated when the individual feels weak and debilitated and the fistula is not healing properly. It is also useful for individuals with a tendency toward chronic conditions.
Key symptoms: Weakness, fatigue, and a tendency for chronic conditions, such as an anal fistula, to persist or worsen over time.
Asafoetida is helpful for fistulas that are accompanied by gas, bloating, or digestive disturbances. It is particularly useful for those with a lot of flatulence or bloating.
Key symptoms: Fistula associated with bloating, gas, or indigestion; foul-smelling discharge; worse from motion.
Calcarea Carbonica is used when the fistula is accompanied by sluggish healing, particularly in individuals who are prone to infections and have a tendency to gain weight.
Key symptoms: Swelling, induration, or slow healing around the anal area; the individual may feel cold, sluggish, and fatigued; weight gain, especially around the abdomen.
